Almost every treatment in this industry is sold on a guess. Someone points at what bothers them, someone else agrees it can be treated, money changes hands. Nobody measured anything first, and nobody asked how the rest of that person was doing. The Spiral is our answer to both.

It is the mechanism by which we build a protocol: a researched, sequenced plan that moves one person towards one goal, assembled from the evidence rather than from a treatment menu. It runs on eight domains, and it is the same eight whether you came in about your jowls, your energy or the next twenty years.

It spirals outward for a reason.

The line starts small at the centre with the things that cost nothing and widens as it goes. That is the actual order of operations: the free foundations first, because they carry most of the weight, and the clinic work last, because it accelerates a foundation that already exists.

    What you eat turns up in your skin about six weeks later. We look at protein, blood sugar and how you cook rather than handing you a diet sheet. Nothing is banned, and nothing here needs a subscription.

    Supplements

    Most of what is sold in this category is wasted money. Our research team grades the evidence, so you are told which few are worth buying and — more usefully — which to stop. Anything prescription-only is discussed with a doctor in person.

  3. Free

    Connection

    The domain people are most surprised to find in a skin clinic. It covers who you actually see in a week, and whether you have anywhere to be honest. It is the one our patients most often score worst on.

    Recovery

    Stress, sunlight and how you actually wind down. This is where sun protection lives, along with breathing, heat, and whatever genuinely switches your head off — which is different for everybody.

    Not a training plan. The dose of movement that fits the week you really have, weighted towards the kind that suits your goal rather than the kind that is fashionable.

    The domain with the largest effect and the least glamour. We look at consistency before duration, because a regular imperfect night beats an irregular long one.

A worked example

Twelve months of a skin plan, in the order it actually happens.

The Spiral is easier to judge when you can see one. Below is how a plan gets built for the most common reason people come to us — skin that has started to look tired — from the first assessment through to the twelve-month review.

This is an illustration of the method, not a patient. We are a new clinic and we will not invent a case study or a result we have not measured. When we have twelve months of our own outcomes, this section becomes a real one.

Where it starts

“I just need people to stop saying I look tired. I don’t like the dark circles, I hate my jowls, and there’s this annoying redness round my nose. I don’t want to look done — I just want to look like me on a good day.”

Then everything is scored, and the list re-orders itself

Every candidate move is scored out of ten for how easy it is to actually do, and out of ten for how much it will change the result. Multiply the two and you get a priority out of a hundred. It is deliberately blunt, and it produces an uncomfortable answer for a clinic: five of the top seven cost nothing.

Scores are illustrative of how we prioritise. Yours come from your own assessment, and the ease score is personal — a move that is easy for one person is not for another, which is the entire point of scoring it.

Why the free things keep winning

Because how strong the evidence is and how strongly we recommend something are two different questions. The first asks “is this true”. The second also has to weigh what it costs you, how much effort it takes, and what happens if it does not work. A free, reversible move with decent evidence can outrank an expensive one with excellent evidence and a small effect. That is arithmetic, not modesty.

And then it is released one phase at a time

Nobody changes eight things at once. The plan arrives in order, and each phase has to be running before the next one is added — which is also why the clinical work sits where it does rather than at the start.

Movement, plants, and the first device

Aerobic work three times a week and resistance twice, dosed to the week you actually have. Thirty or more different plant foods weekly, and cooking below 120°C where it is easy to — gentler heat produces fewer of the compounds that stiffen collagen.

At-home LED starts here, once there is a foundation for it to work on. Red and near-infrared, three to five times a week.

The first clinical work — two months in, not on day one

A course of RF microneedling: three sessions, four to six weeks apart. It works three ways at once — a controlled mechanical injury, radiofrequency heat, and the healing response that follows — and it is the point at which the change becomes obvious to other people rather than just to you.

It is scheduled here on purpose. Run it on five hours’ sleep and unmanaged stress and you pay full price for part of the result.

Re-scan, then decide what is actually worth adding

You are photographed and scored again against your baseline, so nobody has to rely on memory. What has moved tells us what to keep, what to drop, and whether anything further earns its place.

For some people that is a biostimulator, which prompts your own collagen over eighteen to twenty-five months rather than filling anything. For others it is nothing at all, and we say so.

Supplements, if any, get considered here rather than at the start — usually omega-3, sometimes collagen peptides, always with your GP in the loop and with honest framing about how modest the evidence is.

And what would not be in it

A plan is defined as much by what gets left out. These are current positions, and they change when the evidence does.

×A treatment the market is selling hard right now

There is a regenerative category being promoted heavily across the industry at the moment. The underlying biology is genuinely interesting. The products being sold are not standardised and not consistently regulated, so there is no agreed answer to what is actually in them. We are watching it closely and not offering it.

×Percentages we cannot stand behind

Plenty of impressive figures circulate in this industry, including in our own older material. If a number has not been traced back to the study, the population and the timescale it came from, it does not go in front of a patient. You will notice this page is light on statistics.

×Everything at once

The plan above has four phases across twelve months, not a list of thirty things handed over on day one. Health advice fails on adoption rather than on content, and a plan you abandon in March is worth less than a smaller one you keep.

How we decide what is true

We employ our own researchers. Most clinics read the brochure.

Aesthetic medicine has a weakness for the impressive-sounding. A supplier makes a claim, it gets repeated at a conference, and within a year it is being sold on high streets as though it were settled.

So we do the reading ourselves. Our research team works through the published evidence behind every intervention we might put in front of you — what a study actually measured, how many people were in it, how long it ran, who paid for it — and grades how strong that evidence really is. Then everything is sorted into one of three piles.

Offered with honest expectationsPromising, but the evidence is thinner than the marketing. We will tell you it is a reasonable bet rather than a certainty, and what it costs to find out.

Left outMechanism without outcomes, tiny studies, or research funded by whoever sells it. Plenty of things we could comfortably have sold you live here.

It also means we will tell you when the honest answer is “the evidence is not there yet”. You should expect that from anyone in healthcare. In this industry you rarely get it.

The Spiral is not something we do behind the scenes. It is the shape of your appointment.

We find out what you actually want

Not which treatment you would like. What you are hoping will be different — in the mirror, at four in the afternoon, and in how you would like the next ten years to go. Everything afterwards is measured against that.

We measure, so there is a starting point

A diagnostic camera scores your skin — texture, redness, pigment, the sun damage sitting under the surface. Then we ask you to score yourself. The two rarely match, and the gap teaches us a great deal about what you are really here for.

We read all eight domains

Sleep, food, sun, movement, stress, connection, and what you are already doing at home. This is the part other clinics skip. It is where most of your twelve-month result is decided, and it is the reason a skin appointment here ends up improving things that have nothing to do with your skin.

You get one plan, in order

Not a list of everything possible — a sequence, starting with whatever moves the needle furthest for the least effort. Free things first, because they carry the most weight. Clinical treatments where they genuinely earn their place, and not before.

We check whether it worked

You were measured at the start, so in six months nobody has to rely on memory or optimism. If something is not doing what we expected, we change it and say so.

The part that is not science

Connection is one of the eight.

People are always surprised to find it in a skin clinic. It is there because loneliness turns up in your health long before it turns up on your face.

It is also there because most people who sit down with us have not been properly listened to in a long time. You get time here. You get to say what is actually going on, including the parts that have nothing to do with your skin, and the plan is built around your life rather than around our diary.

Between ourselves we describe the job as making happy, healthy humans. It sounds soft next to the grading and the measurements. It is the reason for all of it.

Why we can afford to work this way

This clinic is not how we make our living.

Tim and Miranda train clinicians — doctors and nurses across the world learn how to treat safely, and how to run a clinic properly, from the two of them. Holmes Chapel is where we have to prove it works.

Every protocol on this page is being documented and taught to clinics in other countries. That is the whole point of the place: to raise what patients can reasonably expect from a wellness clinic, and to give other clinicians a worked example they can copy.

It also settles the question you are too polite to ask. A clinic that lives on treatment revenue has a quiet reason to suggest one more thing. We are being watched by the people we teach — so our reputation depends on you getting the right answer, including when the right answer is “not yet”.
